Prepare for Your Visit

All new patients should arrive at least 15 minutes prior to their scheduled appointment to allow adequate time to complete intake paperwork. Please bring identification, insurance card(s), list of current medications and payment for any applicable copays, coinsurance or deductibles. 

If you’re coming for a skin check or procedure please wear loose, comfortable, easy-to-remove clothing. Avoid clothing with multiple layers, buttons, collars or zippers. Gym clothes are a great choice.
